Payment and Billing

Family Medicine of Port Angeles is contracted with most major insurance companies and will bill them directly for services provided. To facilitate this process, please bring your insurance card to each visit. If we are not contracted with your insurance plan, or if you are not insured, please be prepared to pay at the time of service. An estimate of costs can be provided when you schedule an appointment. For your convenience we accept Visa, MasterCard, and personal checks.
We strive to keep health care costs down by eliminating financial processing and billing expenses. For this reason:

  • Please be prepared to pay your bill at the time of service
  • Please be prepared to pay your co-payment and deductible at the time of service.
  • For services not covered by your plan, please be prepared to pay at the time of service.

If you are unable to pay for a service at the time it is provided, our accounts representative will discuss possible billing arrangements before your appointment. For all accounts past 30 days due we charge a rebilling fee of $5.00 per month to cover billing expenses.

As a Rural Health Clinic, Family Medicine of Port Angeles is able to offer most services on a sliding fee schedule. This means that depending on your family income and family size, you maybe be eligible for fee discounts. Click here for more information.
