Payment and Billing

Family Medicine of Port Angeles is contracted with most major insurance companies and will bill them directly for services provided. To facilitate this process, please bring your insurance card to each visit. If we are not contracted with your insurance plan, or if you are not insured, please be prepared to pay at the time of service. An estimate of costs can be provided when you schedule an appointment. For your convenience we accept Visa, MasterCard, and personal checks.
We strive to keep health care costs down by eliminating financial processing and billing expenses. For this reason:

If you are unable to pay for a service at the time it is provided, our accounts representative will discuss possible billing arrangements before your appointment. You may be asked to complete a financial statement. For all accounts past 30 days due we charge a rebilling fee of $3.00 per month to cover billing expenses.
Please ask our receptionist if you would like a copy of our complete office payment policy. Our billing department is available by telephone at (360) 452-8086 or in person at our 8th Street office to answer any billing questions or concerns that you may have.
