Sliding Fee Program

As a Rural Health Clinic, Family Medicine of Port Angeles is able to offer most services on a sliding fee schedule. This means that depending on your family income and family size, you may be eligible for fee discounts.

Sliding Fee Schedule Eligibility:

The sliding fee schedule program is for established patients only. Our billing staff at Family Medicine of Port Angeles is available to assist patients with determining if they are eligible for discounts via the Sliding Fee Schedule Program. Patients who do not have health insurance and meet the necessary application requirements will receive the discounts. You may be required to supply a denial letter for medical assistance through DSHS (Department of Social and Human Services) before being considered for the sliding fee program. We use the Federal Poverty Guidelines to determine the percentage of discounts available. A schedule will be given to you upon application for the program or you can access the fee schedule and application by clicking here.

 

How to apply for the fee schedule discounts:

Please call our business office, (360) 452-8086 to make an appointment with our Accounts Representative. She will give you an application and a list of information that we need in order to determine eligibility or you can download it here. Once you have applied and supplied all the necessary information, your application will be reviewed for eligibility and the percentage of discount will be determined according to the Federal Poverty Guidelines.
